Both are needed. Evidence without impact is noise. Impact without evidence is a guess.

Evidence visible on the card
The card description should mention the signal in simple words.
It can point to a prompt gap, source pattern, competitor movement, readiness issue, or missing integration.

Good versus weak examples
Good: "Competitors are named from review pages where your brand is missing."
Weak: "You should improve review pages."
The good version tells the user why the action matters.
